Determine the eligibility of clients referred to the program; interpret and assess the client’s needs through home visitations
Recommend alternate resources for clients, as appropriate
Research, explore and identify possible funding opportunities for the continuation of CSS programs
Collaborate with other health professionals inside and outside VON in an open and professional manner
Maintain and update a database of clients with details of recruitment, referrals, assessment and matches
Report to Manager, Community Support Services on a regular basis, providing program updates and reports as requested
Supervision of staff, including administrative and field staff
Convey a positive image of VON through participating in community group activities
Member of the Community of Practice committee and other VON committees as required
Volunteer recruitment and retention, orientation, training and in-services
Assign suitable volunteers to clients according to identified needs, accompany volunteers to the client’s home for the first visit, closely monitor the relationship and ensure that the standards and policies set forth by VON are met (where applicable)
Provide support, resources and feedback to volunteers while enabling them to provide quality service to clients
Responsible for volunteer evaluations and recognition
Perform additional functions as delegated by the Manager, Community Support Services
