Effectively supervises and supports Personal Support Workers and their employment/performance
Interviews and provides orientation to PSWs, in addition to providing ongoing mentoring, training and support in collaboration with the Care and Service Manager or delegate
Plans and participates in multi-disciplinary meetings as required (care plan meetings, team meetings)
Completes assessments (such as interRAI CHA) according to VON policy and client needs and funder requirements
Plans, implements, evaluate and adjusts client-focused care plans according to changing needs of clients and according to VON standards/policies
Promotes client-directed care, self care and independence through teaching
Promotes involvement of client/family in determining health and service needs
Works in collaboration with Client Service Associate or delegate to ensure that client’s scheduled visit needs are met
Performs chart audits to ensure that documentation in client charts are accurate and completed according to VON standards and policies
Timely communication and follow up with clients, staff and community partners/professionals
Ensures Mask Fit Testing is completed for new staff and re-testing according to policy
Completes Disaster and Emergency Drills according to VON Disaster & Emergency program/policy
Participates in health and safety training including WHMIS and applies this knowledge in the workplace
Ensures all workplace incidents are reported in a timely manner in accordance with legislation and VON policies and procedures for workplace injuries and incidents
Demonstrates effective time management, planning and organizing of work activities/responsibilities
Documents clearly, concisely, accurately and appropriately according to VON standards
Support statistical tracking/reporting requirements for VON and funder obligations
Practices in accordance with the quality and risk management framework of VON Canada and Occupational Health and Safety legislation
Other tasks/duties as requested
